Services

Patients Aged 75 Years +

Patients aged 75 years and over who request a consultation and have not been seen by a clinician in the past 12 months, will receive a health check as deemed appropriate by the clinician during consultation.

This consultation will take place in the home of the patient where, in the opinion of the clinician, it would be inappropriate as a result of the patient's medical condition for them to attend the practice premises.

Patients Not Seen Within Three Years

Patients aged 16-75 who request a consultation and who have not seen a clinician within a period of three years prior to the date of request will receive a health check as deemed appropriate by the clinician during consultation.

Test And X-Ray Results

If you have had a blood test you will be asked to ring back for the result within seven days. If you need to be seen before then the surgery will contact you and make an appointment with the doctor. X-rays take between 10-14 days for the results to come through, but if you need to be seen before then you will be contacted.

Local Transport

Many buses pass through Cudworth regularly from most of our practice areas.

Information On Other Facilities

We regret that there are no designated baby changing or feeding rooms available.

For our disabled patients the clinic offers a ramp, disabled toilet and specially adapted doors.

If a patient needs translation while having a consultation with one of our GPs, the doctor will ring a specific number and give our caller ID with the language needed and the translator will speak to the patient while in surgery.

Responsibilities

Choice Of Practitioner
Patients are registered with the practice rather than individual GPs. However, patients may request to be seen by a practitioner of their choice. Where a patient wishes to exercise this right, the patient may have to wait longer to see their preferred practitioner.

Violent Or Abusive Patients
The practice will not tolerate violent or abusive behaviour. Anyone verbally abusing either a member of staff or the public, or using inappropriate language, will be asked to leave the premises and requested to find another GP.

Anyone who is violent or causing damage will be removed from the list immediately.

General Information

Most of our patients (approx 7000) are registered at our main surgery with 2400 at Monk Bretton surgery. As part of our ongoing development for the practice and the services we provide to our patients we have moved to new premises at Cudworth as part of the LIFT (NHS Local Improvement Finance Trust) initiative.

In our practice development plan we have identified coronary heart disease as being a major area, together with respiratory diseases and asthma in the under five years age group.

Our advance access arrangements continue apace and we have also been successful in lengthening our appointment times and trying to have fewer face-to-face consultations, due to triage and over-the-telephone consultations.

We also make good use of PRIMIS programme to improve patient care through effective use of our clinical computer system.

Having a fifth doctor as a salaried doctor or partner and nurse practitioner enables the practice to increase surgery hours and choice which will lead to improved access arrangements, appropriateness and quality of intervention and enhance the patient experience. It is also anticipated that this will lead to a reduction of pressure on practice staff and enable time to be assigned to developing the educational skills and workforce.

Welfare Rights Officer

Julie Marshall is our welfare rights officer and attends every Tuesday and Friday between 1.30 - 3.30pm. If you hate filling in forms and don't know who to ask, make an appointment at the reception desk. You could be missing out on money or information to make your life better.

Freedom Of Information - Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available. This scheme is available from reception.
